This field is intended for a brief but informative description of your request.
📎 What to Include in the Reason
This field is mandatory when booking a consultation. You can indicate:
- Symptoms that concern you
Example: headache, fever, cough, rashes, discomfort during urination - Request to extend previously prescribed treatment
Example: need to extend a previously prescribed medication - Follow-up on a previously discussed issue
Example: continuing monitoring and adjustment of high blood pressure treatment - Questions about health or lifestyle recommendations
Example: fatigue, low energy, desire to discuss healthy lifestyle and preventive measures
